How it’s harvested and why the quills look different
Harvesting Ceylon is skilled manual labor. Workers scrape the outer bark off young shoots, bruise the stem to loosen the inner bark, peel it in long strips, then nest those thin sheets inside one another as they dry and curl. The result is a quill with visible layers, papery and tan-brown, light enough that it crumbles between your fingers.
Cassia is a single thick curl of bark. Reddish-brown, dense, and hard enough to genuinely damage a cheap spice grinder (I’ve done it). If you can’t crush a stick with your thumbnail, you’re not holding Ceylon. That single test is more reliable than most product labels.

Take the person away from the coumarin and liver enzymes normalize. It isn’t a horror story. But it isn’t nothing either.
The European Food Safety Authority and Germany’s Federal Institute for Risk Assessment (BfR) both landed on a tolerable daily intake of 0.1 mg of coumarin per kilogram of body weight per day. Germany got serious about this in 2006 after Christmas baking season produced cinnamon star cookies with coumarin levels that made regulators wince.

Cassia bark typically contains somewhere between 1 and 12 mg of coumarin per gram. That range isn’t sloppy reporting, it’s real batch-to-batch variance, and it’s worst in C. burmannii. Blahová and Svobodová surveyed cinnamon sold on the Czech market and found spreads wide enough that two jars off the same shelf could differ by an order of magnitude.
Ceylon cinnamon sits consistently under 0.02 mg per gram. In plenty of analyses it’s below the detection limit entirely.
That’s not a modest difference. That’s a factor of several hundred.
Doing the math on your own daily limit
Let’s run it properly, because this is the part everyone skips.
Take a 70 kg adult (about 154 lb). The TDI is 70 × 0.1 = 7 mg of coumarin per day.
One level teaspoon of ground cinnamon weighs roughly 2.6 grams. If that teaspoon is Cassia at, say, 3 mg/g, you’re at 7.8 mg. Already over. At the high end of the range, one teaspoon delivers north of 12 mg. Two teaspoons a day of a bad batch and you’re at triple the tolerable intake, every day, indefinitely.
Now the same math with Ceylon. At 0.02 mg/g, a teaspoon gives you about 0.05 mg. To reach 7 mg you’d need roughly 140 teaspoons. Per day. Nobody is doing that.
That single comparison is the entire practical argument for Ceylon cinnamon. Not flavor, not romance about Sri Lankan hand-peeling. Math.
When Cassia is still fine (and when it isn’t)
Here’s a nuance almost nobody mentions: coumarin is fat-soluble and poorly extracted into water. Which means a properly water-extracted Cassia supplement (the Cinnulin PF style of product) carries a genuinely different risk profile than Cassia powder. The polyphenols come across into the water. The coumarin largely doesn’t. If a supplement says “aqueous extract” and the manufacturer publishes coumarin testing, the species question softens considerably.

Cinnamaldehyde
Cinnamaldehyde is the headline compound, making up roughly 50 to 63 percent of Ceylon bark essential oil. It’s responsible for the smell, most of the antimicrobial activity, and a good chunk of the metabolic effects seen in animal work. Honest caveat: Ceylon contains a lower absolute cinnamaldehyde concentration than Cassia in several comparative analyses. Milder flavor isn’t just perception. You may genuinely need slightly more powder to hit the same compound load.
Type-A proanthocyanidins and MHCP
This is where the interesting chemistry lives. Anderson’s group at the USDA isolated doubly-linked type-A procyanidin polymers from cinnamon and showed in J Agric Food Chem (2004) that they increased insulin-dependent glucose uptake in cell culture. Before that, Jarvill-Taylor and colleagues (2001) had identified MHCP, methylhydroxychalcone polymer, as the original “insulin-mimetic” fraction, the compound that kicked off two decades of cinnamon research.
Both are water-soluble. Both survive into aqueous extracts. Neither is coumarin.
Eugenol, cinnamic acid, and the rest
Ceylon bark carries notably more eugenol than Cassia (eugenol being the dominant compound in clove), which is a large part of why it tastes sweeter and rounder rather than hot and aggressive. Add cinnamic acid, cinnamyl acetate, linalool, and a long tail of minor polyphenols.
One thing to hold onto before we get to the trials: whole ground powder, hydroalcoholic extract, aqueous extract, and essential oil are four chemically distinct products. They have different compound profiles, different absorption, and different evidence bases. Treating them interchangeably is how supplement marketing gets away with murder.
How Does Ceylon Cinnamon Actually Work?
Making insulin receptors more responsive
Imparl-Radosevich and colleagues published work in 1998 showing that cinnamon extract increased autophosphorylation of the insulin receptor beta-subunit while inhibiting PTP-1, the phosphatase whose job is to switch insulin signaling back off.
Think of the insulin receptor as a door with a sticky lock. Cinnamon polyphenols don’t hand you more keys (they don’t raise insulin). They oil the lock so the key you already have turns more easily. That’s the cleanest one-sentence version of the mechanism, and it’s why the effect shows up mostly in people with insulin resistance and mostly vanishes in people without it.
Slowing carbohydrate digestion
Cinnamon extracts inhibit alpha-amylase and alpha-glucosidase, the enzymes that chop starch into absorbable glucose. Slow those down and the postprandial spike flattens. It’s the same broad mechanism acarbose uses, just far weaker. Nobody should confuse a teaspoon of bark with a prescription drug, but the direction of effect is genuine.
Slowing gastric emptying
Hlebowicz et al., writing in the American Journal of Clinical Nutrition (2007), gave healthy subjects 6 g of cinnamon with rice pudding and measured both gastric emptying and glucose response. Emptying slowed. Postprandial glucose dropped.
Then the same group ran the follow-up at 1 and 3 g. Nothing. No significant effect at either dose.
That null result deserves more attention than it gets, because it puts a floor under the dosing conversation. If 3 g does nothing to gastric emptying in healthy people, the “sprinkle it on your latte” advice is mostly folklore.
GLUT4, AMPK and the cell-level story
In rodent and cell work, cinnamon compounds upregulate GLUT4 translocation (GLUT4 being the transporter that actually drags glucose into muscle and fat cells) and activate AMPK. Think of AMPK as your cell’s low-fuel warning light: when it fires, the cell switches from storing energy to burning it. Cinnamaldehyde also activates TRPA1 receptors, which is a plausible route for the mild thermogenic and gut-motility effects people report.
The caveat I won’t bury: nearly all of this mechanistic work used cell cultures, rodents, or concentrated Cassia extract. Going from “activates AMPK in mouse adipocytes” to “a teaspoon in your coffee will do something” is a substantial leap. Mechanism tells you a story is plausible. Trials tell you whether it’s true.
So what happened when people actually ran the trials?

The 2003 study that started everything
Khan et al., in Diabetes Care (2003), took 60 people with type 2 diabetes in Pakistan and gave them 1, 3, or 6 g of Cassia cinnamon daily for 40 days. Fasting glucose fell 18 to 29 percent. Triglycerides dropped 23 to 30 percent. LDL fell 7 to 27 percent. All three doses worked, with no clean dose-response.
Those are drug-sized numbers from a spice. The study made global headlines, sold an enormous amount of cinnamon, and remains the single most-cited paper in this field.
It also has never been cleanly replicated at that magnitude. Not once.
The meta-analyses that disagree with each other
Baker and colleagues pooled five trials in 2008 and found no significant effect on HbA1c, fasting glucose, or lipids. Flat null. Whiplash for anyone who’d read the 2003 paper.
Five years later, Allen et al. in the Annals of Family Medicine (2013) pooled 10 randomized trials covering 543 patients and got a very different answer: fasting glucose down 24.6 mg/dL, LDL down 9.4 mg/dL, triglycerides down 29.6 mg/dL, HDL up 1.7 mg/dL.
Why the disagreement? More trials, longer follow-up, and different inclusion criteria. The Allen analysis is the one I weight most heavily, but its confidence intervals are wide and heterogeneity between trials is high. Translation: the average effect is real, the effect in any given person is a coin flip.
Fasting glucose vs HbA1c: the gap that matters
Here’s the finding that keeps me honest. Allen’s pooled analysis found no significant change in HbA1c. And Akilen’s 2012 meta-analysis in Clinical Nutrition found an A1c reduction of 0.09 percent, which is statistically detectable and clinically meaningless.
How does fasting glucose move 24 mg/dL while A1c barely twitches?
Three reasons. Most trials ran 8 to 12 weeks, and red blood cells live about 120 days, so A1c hasn’t fully caught up by the time the study ends. Second, fasting glucose is one measurement at one moment and it’s noisy, which makes it easier to move on paper. Third, and least comfortable: if the effect were as large as the fasting numbers imply, A1c should show it. The gap suggests the true effect is smaller than the headline.
The Ceylon-specific trials
This is the frustrating part. Ranasinghe’s group in Sri Lanka ran a triple-blind, placebo-controlled trial specifically on Cinnamomum zeylanicum in type 2 diabetes, one of the very few Ceylon-specific human datasets in existence.
Wickenberg and colleagues tested Ceylon cinnamon on postprandial glucose in healthy volunteers and found no significant effect at 6 g. An inconvenient result, and I’m reporting it because too many articles quietly don’t.
The honest summary: the vast majority of positive glucose data comes from Cassia. We are extrapolating to Ceylon on the assumption that the active polyphenols are shared. That assumption is reasonable. It is not proven.
Prediabetes and metabolic syndrome
Effect size tracks baseline. Across the literature, people with higher fasting glucose and higher BMI respond more. People with normal glucose respond essentially not at all, which is exactly what you’d predict from a compound that improves receptor sensitivity rather than adding insulin.
Effect size box: what a 20 mg/dL fasting glucose drop actually means If you start at 130 mg/dL and land at 110, you’ve moved from diabetic range toward the upper end of prediabetes. That’s meaningful and worth having. If you start at 92 and drop to 88, you’ve changed nothing that matters. It does not mean you can reduce medication, it does not reliably move A1c, and it’s roughly what a brisk 15-minute walk after dinner does for the same person. Cinnamon isn’t competing with metformin. It’s competing with a short walk, and the walk is winning on every other endpoint.
Expect a modest nudge. Anyone promising more is selling something.
Cholesterol, Triglycerides And Blood Pressure
The lipid data
Triglycerides are, to me, the most consistent finding in the entire cinnamon literature. Pooled reductions land around 20 to 30 mg/dL across meta-analyses, and unlike the glucose findings, the direction is stable study to study.
LDL reductions are smaller and shakier. HDL barely moves, and the 1.7 mg/dL bump in Allen’s pooled data is inside the noise as far as I’m concerned.
Blood pressure findings
Akilen and colleagues published a blood pressure meta-analysis in Nutrition (2013) and found systolic pressure down roughly 5.4 mmHg and diastolic down about 2.6 mmHg with short-term cinnamon intake.
Context check: 5 mmHg is a real number, the kind that shows up in population-level cardiovascular risk. It’s also almost exactly what you’d get from cutting one gram of sodium a day, and the trials were short.
How it stacks up against other options
I want to be fair to cinnamon without overselling it. For triglycerides specifically, omega-3s at 2 to 4 g of combined EPA/DHA produce reductions of 20 to 30 percent, which outclasses cinnamon by a wide margin. Cinnamon is a supporting player in a lipid strategy, never the lead.
And the heterogeneity problem hangs over all of it. Baseline lipids, dose, species, extract type, and trial length vary wildly from study to study. When you pool studies that different, the average number you get out is real in a statistical sense and slippery in a practical one.
Which brings us to the questions that actually decide whether this is worth your money: how much, what form, and what the FDA found in cinnamon powder in 2024.

Antioxidant And Anti-Inflammatory Effects
ORAC scores and what they’re worth
Cinnamon sits near the top of every spice ranking for polyphenol content per gram. Marketing departments love that fact. I’m less impressed, and here’s why: the USDA quietly pulled its ORAC database in 2012 because in vitro antioxidant capacity turned out to be a terrible predictor of what happens inside a human being.
A compound that scavenges free radicals in a test tube may never reach the tissue that needs it, may be degraded in the gut, or may act through entirely different pathways once absorbed. ORAC measures chemistry in a dish. You are not a dish.
The inflammatory marker data
The human data is better than the ORAC pitch, though still modest. Trials in metabolic syndrome and type 2 diabetes populations have reported drops in hs-CRP, reductions in malondialdehyde (a marker of lipid peroxidation), and increases in glutathione and total antioxidant capacity. One 12-week randomized trial in people with type 2 diabetes found lower serum MDA alongside higher total antioxidant capacity.
Mechanistically, cinnamaldehyde suppresses NF-kB signaling, which is the master inflammatory switch in a cell. That’s well documented in cell culture. Much less so in people at realistic doses.
My honest take? The antioxidant angle is the weakest claim on the bottle, and it’s the one nearly every brand leads with. If you’re buying cinnamon for “antioxidants,” buy blueberries instead.
The Other Claims: PCOS, Gut, Brain, Weight And Infections
PCOS and menstrual regularity
This is the area that surprised me most. Kort and Lobo, publishing in the American Journal of Obstetrics and Gynecology (2014), randomized women with PCOS to cinnamon or placebo and found improved menstrual cyclicity over six months. Not a marker. An actual clinical outcome women care about.
Earlier groundwork came from Wang and colleagues in Fertility and Sterility (2007), a small pilot showing improved insulin sensitivity in PCOS. Since insulin resistance drives a large share of PCOS pathology, the mechanism hangs together. Sample sizes were small. I’d still call this the most interesting under-researched use of ceylon cinnamon on the list.
Antimicrobial and antifungal activity
Cinnamaldehyde and cinnamon bark oil are genuinely potent antimicrobials in vitro, with activity against Candida albicans, Listeria, E. coli and Salmonella, plus documented biofilm disruption. That’s real science with real applications, mostly in food preservation and packaging.
Does swallowing a gram of cinnamon powder treat an infection? No. The concentrations that kill microbes in a petri dish are nowhere near what your bloodstream sees after a teaspoon in yogurt. Useful chemistry, speculative medicine.
Cognitive and neuroprotective research
Frydman-Marom et al. reported in PLoS ONE (2011) that a cinnamon extract they called CEppt inhibited amyloid-beta aggregation and improved cognitive performance in transgenic mice.
Mice. I want to be very clear about that. Mouse models of Alzheimer’s have produced hundreds of “promising” compounds that failed completely in humans. File this under interesting, not actionable.
Weight loss, where I have to push back
No meta-analysis I’ve read shows meaningful change in body weight or body fat from cinnamon. A few show small waist circumference reductions, and those trials almost always bundled dietary counseling alongside the supplement, which makes attribution impossible.
The fat-burner marketing on cinnamon products is fiction. Ignore it entirely.
Oral and gut health
Two emerging areas worth watching. Cinnamon polyphenols behave somewhat like prebiotic substrates, and preliminary work suggests shifts in Lactobacillus and Bifidobacterium populations. Separately, dental researchers have tested cinnamon oil against Streptococcus mutans and oral malodor with decent results, which is why you’ll find it in some natural mouth rinses.
Both are early. Neither is a reason to buy.
The Other Claims: PCOS, Gut, Brain, Weight And Infections
Dosage: How Much Ceylon Cinnamon Should You Actually Take?
Doses used in the research
Clinical trials cluster between 1 and 6 grams of powder per day. The sweet spot for fasting glucose effects, based on the trials that actually found something, is 1 to 3 grams daily.
Above 3 grams, the dose-response curve flattens hard. You get more GI complaints and no more benefit.
The teaspoon conversion nobody prints on the label
One level teaspoon of ground Ceylon cinnamon is roughly 2.6 grams. So half a teaspoon twice a day puts you squarely in the trial range without needing a scale.
Powder vs capsule vs extract (the labeling trap)
Here’s where most people get fleeced. The concentrated aqueous extracts used in research, the Cinnulin PF-type products, were studied at 250 to 500 mg per day. That is a 10:1 concentrate. It is not remotely the same as 500 mg of raw ground bark in a capsule.
Read the ratio on the label:
- “Ceylon cinnamon bark 500 mg” means half a gram of powder. You’d need four to six capsules a day to reach trial doses.
- “10:1 extract 500 mg” is equivalent to roughly 5 grams of raw bark. Very different product.
I’ve seen bottles priced like extracts that contain plain powder. Check the ratio before you check the price.
Timing: does it matter?
Yes, and more than people assume. The two best-supported mechanisms (alpha-glucosidase inhibition and slowed gastric emptying) are both meal-dependent. Cinnamon taken on an empty stomach at 10 pm isn’t inhibiting the digestion of carbohydrates that aren’t there.
Take it with carbohydrate-containing meals. Split doses beat one large dose, both for tolerance and for covering more of the day.
How long before you see anything
Three different timelines, and conflating them is why people quit too early or expect too much:
- Postprandial glucose: same day. If you have a CGM, you can test this at lunch.
- Fasting glucose: 4 to 12 weeks in most trials.
- A1c: 3 months minimum, because that’s how long red blood cells live. Anyone promising A1c movement in 4 weeks doesn’t understand the measurement.
A practical starting protocol
Here’s what I’d actually do. One gram (about half a teaspoon) of ceylon cinnamon with breakfast, one gram with dinner, for eight weeks. Track fasting glucose weekly with a home meter, or run CGM comparisons if you have one.
Don’t exceed 6 grams a day of powder. The gastric emptying and GI tolerance ceiling arrives well before any extra benefit does, and loose stools are a poor trade for a mechanism you can’t feel.
One preference of mine: I’d rather most people use whole ground powder than capsules. It’s cheaper per gram, you can verify the color and smell yourself, and you’ll actually taste whether it’s Ceylon or Cassia. A capsule hides everything.

The coumarin liver concern that dominates cinnamon headlines is a Cassia problem. Ceylon contains trace amounts, often below 0.02 percent, while Cassia can run 200 to 400 times higher. Avoiding hepatotoxic coumarin exposure is the entire reason to pay more for Ceylon.
Reported side effects
The realistic ones, in rough order of frequency:
- Mouth and lip irritation, sometimes contact stomatitis, driven by cinnamaldehyde
- Heartburn or reflux
- Loose stools at gram-level doses
- Allergic contact dermatitis in sensitized individuals
None of these are dangerous. All of them are reasons to start at 1 gram rather than 3.
Drug interactions worth knowing
Additive hypoglycemia is the one that matters. If you’re on metformin, a sulfonylurea, or insulin, cinnamon can nudge glucose lower on top of your medication. Monitor. Do not adjust or stop prescriptions on your own because a spice moved your numbers 8 mg/dL.
